You can request a visit from a trained volunteer visitor who is a heart patient, the parent of a child with a heart problem, or a caregiver of someone with heart disease.

Here’s how:

1. Select a time for your visit. You can choose a visit by:
   • Phone call
   • Video chat* (Zoom)
2. Fill out the form.
3. You will be assigned a Visit Request Number that you should save, or choose to receive by email, text (SMS).
4. You will need your Visit Request Number to make changes to your appointment request.
